_____Session 4 Serving Camp (10:30 a.m.-12:30 p.m.)

Camps are designed to give specific focus on individual positions.
Girls will be divided into groups according to age and ability.
Camp is for grades 4th thru incoming 8th graders

Medical Release/Waiver We as parents or guardians hereby give permission for our child to participate in the Montgomery Gold Volleyball Camp and acknowledge the fact that she is physically able to participate in camp activities. We further acknowledge that we will be responsible for any illness or injury that may occur during the camp. I hereby authorize the staff of Montgomery Gold Volleyball Camp in the event of an emergency situation to obtain for me medical attention according to their best judgment and I hereby waive and release Montgomery Gold Volleyball Camp from any and all liability for an injury or illness incurred while participation in the camp. In the event of an injury to your child, we recognize that First Baptist Church & Montgomery Gold Volleyball Club, and its employees are no way liable for any injuries, medical expenses, or damages.
